Accessing Health Maintenance has changed from View, Health Maintenance in SOAPware 4.x; to Tools, Generate HM Report in SOAPware 5.0.

Accessing Health Maintenance Customizations has changed from View, Customization Wizards, Edit Health Maintenance Protocols in SOAPware 4.x to Tools, HM Rules and Protocols in SOAPware 5.0.

Statement:

SOAPware includes a Health Maintenance Chart Section that delivers the functionality to create  documentation for the encounter in a fashion that also allows for the ability to address preventive and wellness guidelines.

(Note: Much of the documentation within 6.02 and 6.03 is applicable here as well. For the reviewer's convenience, the information is not repeated.)

Implementation Guide for: Documenting and Viewing of Preventive or Disease Management Services:

Prior to Going Live:

List the person(s) that will be a part of the work flow addressing the preventive/disease management items which are called Health Maintenance Rules and Rule Sets during patient encounters.

Verify all staff involved understand the process.

Going Live:

Using a single guideline, plan to address it in one to two patient encounters daily. Add to the number of guidelines and encounters addressed after staff is confirmed to be competent with the process.

Subsequent to Going Live:

The practice workflows are maintained in order to include the viewing and documentation of the Health Maintenance items (preventive or disease management services).

Clinician (or clinical committee) determines the timing for performing patient recall for the rules. The recall generates lists of patients overdue for specific rule/rule sets.

Note:  Person(s) accessing the Health Maintenance Section must be given appropriate security rights by the security administrator in order to be able to do so. The instructions as to how this task is accomplished reside within the security documentation.

Work Flow Demonstrations:

Viewing past due Health Maintenance items

If a patient has due or overdue Health Maintenance items and the chart is opened, a dialog box will be displayed.

Clicking Yescauses the Health Maintenance Chart Section to be opened into view. Within the chart the items due are displayed in red.

The Health Maintenance items due can also be manually retrieved at any time. With a chart open, click on Health Maintenance tab.

Again, all items due or past due are displayed in red.

Documenting preventative/disease management items during encounters

With a patient's chart open click on the Health Maintenance tab.

Click on the Add Addressed Date icon.

Click within the date column associated with the date therule is to be addressed.

Using the drop down arrows and text boxes fill in the documentation/details as appropriate.

Once all items are addressed, click on the Transfer to SOAP note icon.

Click Yes in the Transfer Health Maintenance dialog box.

Documentation of this rule(s) will be placed into the current encounter.

Create a report or list of patients overdue for rules/rule sets and send reminder/recall letter

Click on Generate HM Report.

Click "Rules to find" and select the date using the drop down arrow.

Select either the Rules or Rule Sets.

Click on a rule/rule set to highlight.

Then, click on Select.

A report can be generated listing the patient charts overdue for the selected rule.

The resulting list can be printed (and stored).

Clicking on Report allows for a designated, standardized recall letter (created in the document designer) to be sent to each patient in the recall list.

Clicking on Create completes the action of performing the notification for the patient.
