Decision Support Reference Maintenance (Needs updating)

    SOAPware includes a default suite of authoritative sources for decision support and clinical reference. The licensing includes links to a complete Clinical knowledge Module from Elsevier. The key, core drug rules come from Multum/Cerner. This is also the source for the inclusion of ICD-9CM codes. Licensing to Epocrates provides views of prescribing information that many clinicians prefer. From within SOAPware, unedited CPT codes, from the AMA, are available from the online library for download. All of the unedited CPT codes have a shortcut of cptsw. Any library items not having a shortcut of cptsw can not be considered to represent formats approved by the AMA.

    Practice guidelines for chronic disease and wellness come from a number of sources and all clinical alerts have convenient locations to include references, including links. Most of the updates for clinical decision support are on-line, automatic and immediately available to the users. The data elements used to create documentation, SMARText, can be automatically updated en mass or manually updated, individually, by the user at any time.

    Drug Interactions and Decision Support

    They are updated monthly and are immediately available to the users for updating their local systems. The updating of the individual drugs and prescriptions used in documentation can be done, all at once, by updating the SMARText Items as described later in this document.

    Licensing to Epocrates is included with SOAPware and provides prescribing information. It is also a means for users to narrow lists of meds based for selection based on the patient’s formulary. SOAPware has an area in the Rx Manager to designate the formulary so that this becomes the default when viewing information in Epocrates.
    MD-Consult can provide more extensive drug information and is discussed later in this document.

    Clinical Alerts Manager:

    The Clinical Alerts Manager allows for the user to administer the rules behind their choices for their decision support system. The implementation guide includes instructions as to how the user may properly administer this clinical alert system. Alert choices include the details referred to as Source/Reference, Date Edited, Date to Review, and others. The steps to edit and maintain these are included in 6.09.

    Order Entry and Procedure Information

    All the CPT codes are licensed from the AMA and are maintained in the on-line SOAPware library of SMARText items, and is updated yearly. Typically, users initially download the codes they commonly use and occasionally go back to the library to download others as they are needed. All CPT codes that are downloaded into the local system are available for order entry and to trigger clinical alerts and linking to information in the Clinical Knowledge Module. A real-time link to the library is not necessary except for when new data items are needed or when updating. The updating of the individual CPT/Order Entry items used in documentation can be done, all at once, by updating the SMARText Items as described later in this document. Most users are updating their SMARText items at least monthly. Soon after the annual CPT updates, we send an extra reminder to update via email and by announcements in SOAPedia (and RSS if preferred).

    Clinical Knowledge Module

    The CKM provides automatic links to the most relevant medical literature. It is designed to be the mechanism for just in time clinical guidance.

    Integrates Elsevier®’s Clinical Decision Support content with SOAPware ®
    Provides access to FIRST Consult, a web-based clinical information system that gives the provider point-of-care access to evidence-based diagnostic tools and the latest patient evaluation and management findings
    Provides access to MD Consult, allowing providers to search and access clinical information from 51 respected reference books, PDF articles from over 70 premier medical journals and clinics, and peer-reviewed clinical practice guidelines
    Directly integrated into SOAPware in order to get either evidence-based, concise information or extensive searching of the medical literature with just 2 mouse clicks.
     

    Here, we have right-clicked over a diagnosis of Diabetes, Type II.

    If the First Consult menu item is selected, the user is taken to that web site for concise, evidence-based information within seconds.

    If the MD Consult menu item is selected, the user is taken to that web site allowing for extensive searching of the medical literature. it is almost as if there is a medical library in the exam room.

    Updating SMARText data Items

    This updating of ST items takes 10-30 minutes depending on how many ST items are in the user's library of downloaded and customized data items. This automatically applies the annual updates for CPT and ICD codes. Older ST items are replaced with newer ones having the updated, unique code identifiers. To do this, the user simply goes to the SMARText Manager (i.e. press F10) and click on the icon to update ST items. Over time, SOAPware, Inc. is steadily adding an increasing number of unique identifiers (ie. SMOMED-CT, ICPC, ICD-10, etc) to the data items most commonly used in documentation. Each time the user updates, they receive the latest edits.

    Reference Library and User Community

    The SOAPware User Community has online web sites where our user's clinical alert administrators may collaborate with each other and SOAPware as they go through the process of selecting the types of alerts for implementation into their own system and to seek/share guidance with implementation.

    After less than 4 months post launch of the SOAPware wiki,  http://SOAPedia.mysoapware.com was often receiving over 1000 unique visitors daily in early 2008.

    Additionally, the product also has a built-in Reference Library that can contain materials that are either shared among all users or held locally in the system. Also available are RSS feeds to our SOAPedia, Forums and Developer Blogs to assist with collaboration.

    The Reference Library is approximately midway down in the Tools menu.
