This page is a fairly detailed demo of how SMARText items can be found and entered into documentation. Presented are a series of pick lists intended to illustrate the characteristics of available pick lists when using SOAPware. None of this is necessary to understand in order to initially use SOAPware, so this is probably not the best place for SOAPware novices.

These demo screen shots were created using a fairly fresh intall of SOAPware, so there had not been a lot of entry of diagnoses in the Assessment field. In the image below, the Assessment field has been entered causing a list of previously used items (in the Assessment field) to be presented on the right in the SMARText Quick Access List. Notice that Sinusitis has been used 7 times, Hypertension 5 times, etc. in the Assessment field.

Over time, as you use SOAPware, the items you are likely to need will start to automatically appear near the top of the Quick Access list. Thus, the need to remember and type code-words is minimized. The design is for users to be able to mostly be able to point and click within SMARTlists in order to more quickly create detailed documentation. With time and usage, the pick lists get smarter. Until the lists get smart, just type the first few letters of any word you wish to use.

Taking a step back, SMARText items can have shortcuts that allow for the direct entry of documentation items without using pick lists (i.e. just as using code/macros in version 4). As an example for this demo, we entered the Assessment field...  typed fibrom ...  and pressed space bar to get...

In other words, typing fibrom + space bar created Fibromyalgia ICD#729.1 

For the next part of this demo, we are going to type fibrom in the Assessment field and not press the space bar. Instead, we are going to review the types of pick lists available by pressing either the F11 or Shift +F11 keys instead. With each of these 2 key commands, we have the option of showing or not showing unused items. SMARText items that have never been used (i.e Unused) have a usage score of 0. If there is not a check in Show Unused, items with a usage score of zero will not be displayed in the pick list even though they may have a keyword matching fibrom.

 

-    Type fibrom in Assessment, and then  press F11 with no check in Show Unused.

Suppose we want to enter a diagnosis. We click in Assessment. If the diagnosis item we want is yet in the QA list, then  we could type the first few letters of the diagnosis and press F11.  (Note: at the time all the series of following screen shots were taken, Fibromyalgia had been used 2 times in the past and had a usage score of 2 within the Assessment field)

Note: Results on your system will be slightly different based on what items are in your local database and due to the evolution of SMARText items in the library over time.

This results in the retrieval of 4 SMARText items in the ST pick list. These are the items that have

     - a keyword matching fibrom, and  

     - are of the Structured Dx item type, because that is the type matched to the Assessment field and given usage score of 1, automatically. In other words, when in the Assessment Field, 1 point is automatically added to all Structured Dx SMARText Items.

    - are in the local database, (i.e. F11 was pressed and this only searches locally)

When searching in the Assessment field, all SMARText items that are of the Structured Dx type will automatically have 1 point added to their usage regardless of whether or not they have ever been used. Because we don't have a check in Show Unused, this effectively limits the pick list items to the Structured Dx type that have had 1 added automatically and that match fibrom.  We should note that Fibromyalgia has a usage score of 3 because it had been used twice in Assessment in the past and then has another point auto-added because it is a Structured Dx SMARText item type.

 


-    Type fibrom in Assessment, and then  press F11 with a check in Show Unused.

This results in the retrieval of 9 SMARText items in the pick list. These are the items that have

     - a keyword matching fibrom, and are of any SMARText item type

     - are in the local database, because F11 was pressed instead of Shift + F11

Because we have placed a check in front of Show Unused, this will allow any/all ST item types to be displayed even if they have not been used in that field and have a usage score of 0. Notice this has caused other item types such as Chief Complaints, history findings, and exam findings to be displayed. Therefore, the default is to not have a check in front of Show Unused. This effectively narrows the list to pertinent item types for the active field. (Unlike in versions prior to SW-2008, where there was often extraneous ST item types within pick lists).

 


-     Type fibrom in Assessment, and then press Shift + F11 and no Show Unused

This follows the same rules as above so the original 4 pick list items are here. However, the additional action of also pressing the Shift key with the F11 key extends the search to the online library.

Note, there are 16 items in the online library that have keywords with fibrom in them. Several of these are not Structured diagnosis item types.

 


-    Type fibrom in Assessment, and then press Shift + F11 and Show Unused

Again, this follows the same rules as above, but adding a press of the shift key to F11 extends the search to the online library and results with many more items of any type that have keywords matching fibrom.

  Summary of options: Type first part of a desired word, then...

-    F11 and Show Unused
-    Shift +F11 and no Show Unused
-    Shift + F11 and Show Unused

Demonstration as to how the presence of a Chief Complaint or Reason for Encounter (RFE) affects the nature of the pick lists. 

 

In the next part of this demo, the entry of a Chief Complaint SMARText item will be demonstrated. Subsequently, we will illustrate how the presence of a chief complaint affects pick lists.

Enter the Subjective field, type ChiX and press the spacebar. This enters CHIEF COMPLAINT(S) into the subjective field. Clicking on CHIEF COMPLAINT(S) produces the pick list seen on the right side in the image below. Notice the pick list is a list of systems.

Finally, clicking in the box in front of Muscle pain enters that into the subjective field. Now, we will go through the same sequences of presenting pick lists with F11, Shift + F11 with and without Show Unused checked. Note: for most of the SMARText items of the Chief Complaint Type, we have added RFE (Reason For Encounter) to the front of the description.

 

 

Differences in pick lists when a Chief Complaint or Reason for Encounter (RFE) is in Subjective

The presence of a Chief Complaint SMARText item type in Subjective causes some significant changes in the Quick Access list for the Assessment field. Below, note that Structured Dx items that are very closely related to the chief complaint have 1000 points added to their usage scores. Items that are within the same system (i.e. Musculoskeletal) have 3 additional usage points added. These additional points are to bring more closely related items more consistently to the top of the list.      The methodology to identify the relationships is via the use of SWICPC codes and does not need to be understood.
